'TICC' has secured more than €4.8 million of European funding in this €8 million cross-border partnership involving 14 organisations from the UK, France, the Netherlands and Belgium (see below for partner details).
The project was approved and funded by the EU Interreg 2 Seas Programme 2014-2020 (co-funded by the European Regional Development Fund) which has supported the project over four years.
TICC will create systemic change in health and social care, providing services better suited to our ageing population by addressing holistic needs. It will present a methodology to overcome blocking points in transferring socially innovative service models from one area to another. This will be tested via the implementation of the Buurtzorg integrated care at home model which consists of self-managing teams of 12 staff working at neighbourhood level handling every aspect of care & business. This model significantly reduces the back office, simplifies IT & coaches rather than manages, providing better outcomes for people, lower costs, fewer unplanned hospital admissions & consistency of care.
TICC will enable other health/social care organisations to implement new ideas; increase staff productivity, recruitment and retention as well as improving patient satisfaction and decreasing costs, emergency admissions and staff absences. It aims to postpone the moment when residential or end of life care is needed.

Peter is leading the reimagine team working / transforming integrated care in the community programme (TICC) for the trust. He has over 25 years’ experience delivering service improvements in healthcare, local government, business and voluntary sectors and is passionate about working with and empowering service users, patients and frontline staff to deliver more effective services.
A project manager supporting EU funded projects and deputy to Peter Brook, Programme Manager, Charmane has previously worked with the finance office of HMRC and Public Liaison Officer for Eurovia Infrastructure in Maidstone.
With more than 20 years’ experience in health and social care, and more than 15 years’ experience in pastoral care settings, David has worked for the NHS, not-for-profit, private and charitable organisations. He is a registered adult nurse, and draws from his varied experience of working with people in different settings to bring a unique contribution to the trust.
Clare is the lead facilitator and joined the reimagine team working support team after being involved with community nursing in Kent since 1996. She was involved in the pilot team based in Edenbridge and visited a Buurtzorg team in the Netherlands to see how they work.
Previously the trust’s Patient Experience Manager for 11 years, Helen enjoys working alongside clinical teams and strongly believes in a correlation between nurse satisfaction and patient outcomes – the notion of “happy nurses, healthier patients”.
Elaine has worked at Kent Community Healthcare NHS Foundation Trust for four years and has been part of the NHS since 1998. She joined the reimagine team in 2020 and helps to find solutions to challenges and barriers that our teams face.
