Who we are

We are a multi-disciplinary team that includes nurses, occupational therapists, physiotherapists, speech and language therapists and care managers. We carry out a full social care and nursing assessment that may include blood tests, observations and wound dressings. We assess and help patients achieve maximum independence in daily living activities, improve memory, concentration and ability to solve problems.

The team can assess for and provide suitable equipment and home adaptations to increase independence. We assess and devise individual exercise programmes to improve muscle strength, mobility and balance to improve abilities to perform daily activities, such as walking, climbing stairs and using limbs. We can assess the need and eligibility for supportive services and, if necessary, give advice and information on issues, such as benefits.

The team can also give advice and information on nutrition and diet to assist in recovery if a patient’s weight has altered, or if they have a poor appetite or swallowing difficulties. Difficulties with communication and/or eating and drinking can be assessed by our team. We have teams in Ashford, Canterbury, Deal, Dover, Folkestone, Herne Bay, New Romney, Tenterden and Thanet.

The Intermediate Care Team also provides an out-of-hours community nursing service, which gives nursing care to all patients in the community between 5pm and 8am. It can be contacted through Integrated Care 24 – please see how you can contact us.

What we do

We provide short-term health and social care support and/or rehabilitation to people in the community. We aim to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care.

We aim to enable patients to live as independently as possible, to continue rehabilitation in the community following discharge from hospital, to refer on to other agencies and professions as needed and to actively involve patients and family/carers in all aspects of care.

In some places, we provide an enhanced intermediate care service, which operates in different ways in different places. For example at the William Harvey Hospital in Ashford, and at the Kent and Canterbury Hospital in Canterbury, we have a presence in the accident and emergency department from 8am to 8pm.

In Thanet, we attend the accident and emergency department at the Queen Elizabeth the Queen Mother Hospital, Margate, from 8 to 8.30am and 3 to 3.30pm Monday to Sunday, as well as liaising with bed and patient flow managers from Monday to Friday. We also provide rapid response to accident and emergency and attend within two hours of referral.
