The Home Treatment Service runs seven days a week from 2 to 10pm, with patient reviews held every morning from 9am to 1pm.

Who we are

We are a community team made up of a consultant geriatrician, speciality doctors and advanced health care assistants. The Home Treatment Service (which replaces our Enhanced Rapid Response Service) forms part of a wider urgent care service.

We also work in line with our partners and other NHS trusts.

What we do

We provide urgent care for up to seven days to patients in crisis. Patients will be seen within two hours of a referral and, when needed, will be transferred to KCHFT’s community services for rehabilitation, long-term treatment or community support.

 

The service’s ultimate aim is to avoid patients being admitted to hospital by diagnosing, assessing and treating patients in their homes. The Home Treatment Service runs as a virtual ward, with patients under the care of a consultant geriatrician.

The service works closely with the hospitals’ discharge teams and GP practices to support early discharge of patients and provide a step-down service for those who are medically fit.

You need to be referred by a GP, healthcare professional to use this service.
