There is a nurse on call 24 hours a day to answer calls and queries.
The team offers a 24 hours out of hours visits for end of life care.

Who we are
We are a community nursing service comprising children’s nurses and children’s support workers. We provide hands-on care and support to children and young people with nursing needs, from short-term illnesses to long-term disabilities or chronic health conditions and end of life care.
What we do
We try to keep children out of hospital as much as possible by delivering care closer to home and helping families and carers to care for their child in the community with our support. We liaise with other healthcare professionals to ensure a seamless service.
We train parents, carers and other professionals and liaise with the multi-disciplinary team to ensure the child’s care is planned appropriately and work in a variety of community settings, including schools, nurseries and patients’ homes.

Ready, Steady, Go, Hello programme
In healthcare, we use the word 'transition' to describe the process of preparing, planning and moving from children's to adult services. To support people with a long term condition through transition, we've adopted the Ready, Steady, Go transition programme.
