Children’s Asthma Team

Who we are

We are a team of people who work with other services, such as the School Nursing Service, health visitors and children’s community nurses, to support children and young people with an identified respiratory condition.

What we do

We work closely with hospital paediatricians to identify children and young people who need support and we work with our partners to care for them.

We provide education and information about respiratory conditions and current treatments to make sure staff work together to meet each child’s needs and our aim it to improve the child’s quality of life as well as helping to control their condition.

We work across east Kent in homes, schools, nurseries, colleges, clinics and hospitals and we run community clinics in:

• Aylesham Health Centre
• Broadstairs Health Centre
• Canterbury Health Centre
• Deal Community Clinic
• Dover Health Centre
• Folkestone Health Centre
• St Stephen’s Health Centre
• Queen Victoria Memorial Hospital, Herne Bay
• Whitstable Health Centre

Medway

In Medway, children and young people who have been admitted to hospital with an asthma exacerbation or viral-induced wheeze may receive follow-up support from the Community Children’s Nursing Team after discharge. The team provides community-based care to support recovery, monitor symptoms, offer asthma education and advice, review asthma action plans and help families manage their child's condition confidently at home.

The service may include:

• follow-up visits at home or in community settings after hospital discharge
• monitoring of respiratory symptoms and recovery
• support with asthma management, including inhaler technique and trigger avoidance
• review of personalised asthma action plans
• liaison with GP practices and specialist services where needed.

Medway referrals

Children and young people are referred directly to the service by the hospital team following discharge after treatment for an asthma exacerbation or viral-induced wheeze. Referrals are not made directly by parents or carers.
