Better at Home: Helping more people receive care where and when they need it most
Most people would rather be treated and recover at home, or closer to home, than spend longer than necessary in hospital. Evidence shows that, when it is safe to do so, people recover more quickly and maintain their independence for longer, when care is provided at home or close to home.
As Kent’s population continues to grow and age, more people are living longer with complex health needs. This brings new challenges for health and care services and highlights the need to develop community-based support, which meets people's needs both now and in the future.
Kent Community Health NHS Foundation Trust (KCHFT) is already providing a range of services that help people stay well at home, avoid unnecessary hospital admissions and return home sooner after a hospital stay. However, as demand increases, we believe there is more that can be done.
To help shape our plans, patients, carers and members of the public are being invited to attend a Better at Home workshop in Herne Bay on Thursday, 16 July from 1.30 to 3.30pm.
The session will focus on proposals to strengthen support for frail older people in the community and improvements in services that help people leave hospital sooner, when they are well enough to continue their recovery at home.
The workshop will give local people an opportunity to share their experiences, discuss what matters most to them and help influence the future development of community services across Kent.
