Medicines Optimisation

The transfer of the Medicines Optimisation Service from the Kent and Medway Integrated Care Network to Kent Community Health NHS Foundation Trust (KCHFT) supports national NHS guidance by bringing together professionals across primary care, community services, pharmacy and hospitals to improve how medicines are prescribed, supplied and reviewed.

The strong links already established with our partners will continue and will support even closer neighbourhood-based working. The aim is to create a more consistent and efficient approach to medicines for patients, the public and partners.

No. GPs and practices can continue to access the same support as they have previously through their named practice leads.

Yes. Your named practice leads will stay the same and you can continue to contact them using the existing contact details.

No. Support for practices will continue without any interruption.

Prescribing queries can continue as normal by emailing the Medicines Optimisation Team.

Formularies and prescribing guidance will remain unchanged and can be foundat Kent and Medway Formulary. Any changes in the future will be communicated by email and through the GP bulletin.

Practices should contact their named practice lead using the existing contact details.
