Who we are
The Children’s Therapies and Care Coordination Team supports children and young people in the community who need specialist help in different areas. Our team includes physiotherapists, occupational therapists, speech and language therapists, therapy assistants, education specialists, specialist nurse advisors and care coordinators.
We work closely with families, schools, and others involved in a child’s life to help them develop their skills and take part in everyday activities. Some children may need help from just one therapist, while others may need support from several professionals working together, coordinated by our care coordination team.
Where do we work from?
The services are provided in a range of venues, depending on the child / young person’s needs as well as local arrangements, these may include:
- multi-agency children’s centres
- community clinics
- pre-school settings
- school settings.
What areas do we cover?
The team covers three main geographical areas within Kent:
- North and west Kent (Maidstone, Malling, Tonbridge, Tunbridge Wells, Sevenoaks, Gravesham, Swanley and Dartford.
- East Kent (Thanet, Canterbury, Canterbury coastal, Sandwich, Faversham and rural Swale)
- South east Kent (Ashford, Folkestone, Dover, Deal and Hythe).
Who this service is for
We aim to make sure our service is available to those who will benefit most. Support is available from: Care Coordination; Occupational Therapy; Physiotherapy and/or Speech and Language Therapy. When children or young people present with needs that require multi-professional support, therapists will work together as part of an integrated team.
Your referral will be reviewed and prioritised by therapists, based on clinical need in a process called Triage. You can expect us to contact you within 12 weeks from us receiving your request for support.
Who this service may not be right for
The Children’s therapy service is designed to support children and young people with specific needs. If your child’s needs don’t fit with what our service offers, we will not be able to provide support at this time. For example, if your child requires a different kind of support or specialist service, we will let you know and, if possible, help guide you to the right place. Please don’t hesitate to ask us or your healthcare or education professional if you’re unsure whether this service is the best fit - we’re here to help you find the right support at the right time.
Role of Children's Therapies
The team work closely with the child / young person’s family as well as in partnership with other agencies including pre-schools, schools, and medical staff. The team can be involved at a number of levels (see below) in supporting children and young people.
Our service works in a tiered approach, recognising children and young people face different challenges at different times in their lives at home, school or in the community. These tiers are called Universal, Targeted and Specialist.
- Universal Support
Universal services are designed to support all Children and Young People, has a focus on prevention and is available to all. It empowers parents/carers, other professionals and education staff to facilitate support for all children and particularly those who may require additional support for their health and education needs. - Targeted Support
Targeted interventions are those that will be provided in partnership with others. Therapists will model and coach others to implement therapy intervention(s). - Specialist Support
Specialist services are for children and young people whose needs cannot be met through universal and targeted provisions. At this level therapists will provide individualised and/or direct intervention(s).
Further information
Further information, including advice on universal strategies to support children’s development can be found on the Children’s Therapies the Pod. Requests for support, including re-referrals for additional support, can be made using the same website.
Support for speech and language therapy is predominantly via our Talking Walk-ins (TIWIs) and School liaison with their School Link Speech & language Therapist. We accept requests from parents and those with parental responsibility, as well as professionals working with a child. If your child is of school age but not in school or you have any concerns about a family or yourselves accessing a TIWI please contact us and we would be happy to discuss this with you.
Support from occupational therapy and physiotherapy needs to be made by a health, education or social care professional using our referral form. We offer a rapid response intervention of up to one month following surgery, botox intervention or hospital discharge as part of our Hospital Discharge and Enhanced Intervention Pathway.
Care Co-ordination team referrals can be made by parents/carers, a GP, health visitor, or any other health, social care or educational professionals.
We operate an online referral form system that can be accessed via Children's Therapies - The Pod. Support may be a requested for one or more professional teams to be involved. Referrals to Occupational Therapy and Physiotherapy can only be made by a health, education or social care professional. We are unable to accept parental/carer referrals for these two disciplines. To help you decide if a referral is appropriate, please see our access criteria below.
There is a separate re-referral form on the Children’s Therapies page if your child has been seen by a therapist in the last 12 months and their case has been closed. This form should also be used if you need to re-refer following completion of a Sensory Processing Difference or Motor Coordination workshop.
Re-referrals need to be for the same reason the child or young person was previously seen for.
Please check the access criteria to ensure the re-request is appropriate for the Children's Therapies re-referral form.
