As your child has now started school, their health and wellbeing will be supported by the School Public Health Service in partnership with you and your child’s school. Some children take the transition to primary school in their stride, others need a little bit more help and we are here to support their individual needs.
To achieve this we are using a new questionnaire called the Lancaster Model, which has been successful in other parts of the country in identifying the extra support some children may need.
Your School Public Health Nurse will send a letter to explain how you can access the questionnaire.
