Who we are

We are a multi-disciplinary team that includes occupational therapists, physiotherapists and therapy assistants.

We help patients leave hospital safely and sooner by providing therapy and support in their own homes.

What we do

We help patients leave hospital safely and support them through rehabilitation, so they can regain independence and feel better in daily life.

Our team can:

  • check if patients need equipment or home adaptations to make daily tasks easier
  • create personalised exercise programmes to improve strength, balance, and mobility
  • set treatment plans focused on patient goals, such as walking, climbing stairs, and self-care
  • give advice on staying safe at home and managing health independently.

We involve patients and their families or carers in every part of care. If needed, we can refer patients to other services or specialists to get extra support.

How do I refer to this service?

All referrals to the TADS team in west Kent are to be made via:

  • an Electronic Discharge Notification (EDN).
