Important Information relating to your dental assessment appointment.
You have been referred for a dental assessment regarding future treatment.
At this appointment a dental assessment will be carried out and the most appropriate arrangements for dental care will be agreed.
Please note: If treatment is not deemed appropriate to be completed here we will work with the dentist who referred you regarding future treatment or ask you to register with a general dental practitioner.
Please note, no treatment will be carried out at this appointment.
We ask that you arrive 15 minutes earlier to the appointment to ensure that all relevant paperwork is completed prior to being seen by the dentist. If you are more than 10 minutes late you may not be seen.
NHS Dental charges will apply unless you are exempt. Please see a breakdown of charges and valid exemptions below.
Please note that some clinics only take card or cheque payments so please check prior to arriving.
If you are currently under the care of a hospital/consultant please bring contact details and a recent clinic letter if available. This will ensure that we are able to gather all information required for your care.
If you wish to re-schedule or cancel your appointment, please telephone 0330 123 9438 leaving a message if there is no one in attendance. A minimum of 48 hours’ notice should be provided to cancel any booked appointment by contacting the relevant clinic. Failure to do this may result in discharge from our care.
Cancellation of two or more appointments may also result in discharge from the dental service if no justifiable reason is provided and a new referral will be required.
NHS Dental Charges
Band 1: £27.40
Covers an examination, diagnosis, x-rays, scale and polish and advice.
Band 2: £75.30
Covers all treatment included in Band 1, plus additional treatment such as fillings, root canal treatment and removing teeth (extractions).
Band 3: £326.70
Covers all treatment included in Bands 1 and 2, plus more complex procedures, such as crowns, dentures and bridges.
