Schools and SENCOs paediatric referral guidance

Who can be referred?

Referrals are accepted for children:

  • Aged 0 to 11 years
  • ADHD referrals: For children aged six to 11 years only.

Conditions the service assesses

The Community Paediatric Service accepts referrals for:

  • Motor disorders
  • Neurodevelopmental conditions
  • Neurodisabilities
  • Physical and sensory medical disorders
  • General developmental concerns (under 11s)
  • ADHD (ages six to 11)*
  • Autism Spectrum Disorder (under age 11)*
  • Developmental assessments for children under five.

*For children aged five and above: a cognitive assessment from school must be included with the referral.

Please ensure all required documentation is complete before submitting your referral:

Background Information Sheet (BIS)

Nursery/school report

ADHD referrals only

Additional supporting information (recommended)

To help clinicians during triage, please include any relevant additional documents such as:

  • Provision plans
  • Risk assessments
  • Educational Psychology (EP) reports
  • Behaviour logs
  • Private diagnosis letters
  • School observations
  • Internal school assessments.

Important reminders

  • All documents must be fully completed before submission.
  • Incomplete referrals may be rejected or delayed.
  • For children over age five, a cognitive assessment is strongly recommended as part of the referral.

Please note: The referral form cannot be saved mid-way, so ensure you have enough time to complete and submit it in one sitting.

Make a note of:

  • Your reference number
  • The submission date
  • The reason for referral.
