Picking people up before they fall

What happens when health, care and community services work as one team around a patient? That is the question being explored by the new Neighbourhood Health Team in Folkestone, Hythe and Rural Primary Care Network.  Bringing together colleagues from different organisations, the pioneer programme is helping people with complex needs to find NHS support earlier.
