The Remote Monitoring service was always set up as a pilot. We have been fortunate that it was able to be extended a number of times beyond its original timeframe. However, it is no longer possible to continue the pilot and it must now come to a close.

No. Your care continues with your GP or other healthcare team you may be supported by. You must contact your GP or healthcare team directly with any health concerns.

If you feel unwell or need advice, please contact your GP direct or call 111. In the case of an emergency call 999.

The pilot ends at 12pm on Friday, 27 March. You will no longer have access to the system from this date.

The devices provided to you as part of the Remote Monitoring pilot are being gifted to you. These may include:

• Smart phone
• Thermometer
• Blood pressure monitor
• Pulse oximeter

Once the pilot has ended, the SIM card in the smart phone will be deactivated and will no longer work for monitoring purposes.

If you would like to continue using the smart phone for personal use, you may do so. Please note that once the pilot has finished, East Kent Health and Care Partnership and East Berkshire Primary Care is no longer responsible or liable for the devices.

No. The system will no longer be active after Friday, 27 March. Any data you submit after this date will not be accessed.

Your data has been handled securely in line with NHS data protection standards. After closure, no new monitoring data will be recorded.
