  • bullying
  • bereavement
  • healthy eating and lifestyle
  • emotional health and wellbeing, including behaviour issues and anger management
  • puberty and growing up
  • sexual health and sexuality
  • smoking
  • drug and alcohol misuse

  • complex health needs.

Health assessments – year 10 and 12

In year 10 (aged 14 to 15) and year 12 (aged 16 to 17) we will send your child a health questionnaire, through the school to help identify any emerging health conditions or health concerns which we can help support. For example, a child may have developed diabetes or asthma, become a young carer, be worried about leaving school or going to college, or have questions or concerns about drugs, alcohol, smoking or relationships.

If you have questions about health assessments contact us on 0300 123 4496 or via
