Transforming Integrated Care in the Community (TICC): project administration privacy notice

This privacy notice is strictly in relation to the administration of the Transforming Integrated Care in the Community Project (TICC), such as our newsletters. This privacy notice is NOT related to the delivery of health and social care services provided to service users under the TICC project.

Who we are

We collect, use and are responsible for certain personal information about you. When we do so we are regulated under the General Data Protection Regulation which applies across the European Union (including in the United Kingdom) and we are responsible as ‘controller’ of that personal information for the purposes of those laws.

The Transforming Integrated Care in the Community (TICC) Project is a 4-year social innovation project seeking to transform the delivery of community care, guided by the principles of Buurtzorg. The project has been approved and funded by the Interreg 2 Seas Programme 2014 – 2020 (co-funded by the European Regional Development Fund). As part of the project we will be sending regular updates on the project via email in the form of a newsletter.

Personal information we collect and use

Information collected by us

Over the course of the Transforming Integrated Care in the Community (TICC) we collect the following personal information about you in the following ways when you provide it to us:

  • during events
  • newsletter sign up
  • the development of case studies
  • photographs and video

We may collect the following personal information via the methods listed above when you provide it to us:

  • name
  • job title
  • organisation
  • email address
  • signature
  • contact telephone number
  • your image via photograph or video
  • your view and/or experiences, which may include special category information, such as health, ethnicity or religious information

How we use your personal information

We use your personal information to:

  • we use attendance lists at all events and workshops so we can see who is in attendance for safety reasons, so in case of an emergency or evacuation we can account for all attendees.
  • send you regular updates of the TICC project and it’s progress by email where you have consented to be part of a distribution list
  • as part of the project we have to demonstrate who we have engaged with and provide evidence of the activities in line with the EU Requirements this is evidenced primarily through attendance sheets, which do not usually go beyond name, role, organisation and signature and is done via Basecamp accessed by all project partners See the list of all project Basecamp's privacy policy
  • as per the project's requirement, all information shared via Basecamp must also be shared with Joint Secretariat; this may include sharing your contact details if requested as part of an audit, however, this would not be used by Joint Secretariat.
  • develop content for our webpages, including the uploading of your image in the form of photographs, videos and case studies
  • create videos and other forms of media (such as pictures)
  • develop promotional material (such as flyers, brochures, posters, newsletters and presentation delivered internally and externally)
  • use as part of our social media and newsletter content

How long your personal data will be kept

For the purposes of audit, we are required to hold all documentation for 5 years following the end of the project closure in 2021. Information supplied will be held internally and may also be stored on Basecamp and by Joint Secretariat as part of our evidence for our claims

Reasons we can collect and use your personal information

We rely on your consent as the lawful basis on which we collect and use your personal data.

We rely on your explicit consent as the lawful basis on which we collect and use your special categories of personal data.

The provision of your contact details and consent is required from you to enable us to send you regular updates in relation to the TICC project in the form of a newsletter via email.

This data sharing enables us to ensure that you receive the most relevant and up to date information regarding the projects progress both locally and internationally. This data sharing also enables us as a project partner to evidence the work we have be doing to meet project deliverables in order to receive funding for the project.

If you no longer wish for us to use the information you have shared with us you can withdraw your consent at any time by notifying us by email or post.

On receipt of your notification we will stop using your information in future publications and social media posts, but it may continue to appear in publications and social media posts that are already in circulation.

We will share personal information with law enforcement or other authorities if required by applicable law.

Your rights

Under GDPR you have rights which you can exercise free of charge which allow you to:

  • know what we are doing with your information and why we are doing it
  • ask to see what information we hold about you (subject access request)
  • ask us to correct any mistakes in the information we hold about you
  • object to direct marketing
  • make a complaint to the Information Commissioner's Office
  • withdraw consent at any time (if applicable)

Depending on our reason for using your information you may also be entitled to:

  • ask us to delete information we hold about you
  • have your information transferred electronically to yourself or to another organisation
  • object to decisions being made that significantly affect you
  • object to how we are using your information
  • stop us using your information in certain ways

We will always seek to comply with your request however we may be required to hold or use your information to comply with legal duties. Please note, your request may delay or prevent us delivering a service to you.

Keeping your personal information secure

We have appropriate security measures in place to prevent personal information from being accidentally lost, or used or accessed in an unauthorised way. We limit access to your personal information to those who have a genuine business need to know it. Those processing your information will do so only in an authorised manner and are subject to a duty of confidentiality.

We also have procedures in place to deal with any suspected data security breach. We will notify you and any applicable regulator of a suspected data security breach where we are legally required to do so.
