Vaccination form - consent or decline your child's vaccination

You only need a few details to complete the form, either your child’s NHS number, or if you don’t have that to hand, their name, date of birth and school name.

Missed an appointment - book an appointment at a community clinic

If your child has missed an immunisation at school, is home educated or you would prefer to have their immunisation outside the school setting, you can book an appointment at one of our community immunisation clinics. 

MMR vaccine catch-up

We are offering a measles, mumps, rubella (MMR) catch-up to all young people who have missed a first or second dose. Those due to have a scheduled vaccine in year eight or 11, can get a MMR vaccine at school at the same time. All other children will need a booked appointment in one of our community clinics. You’ll need to contact us to arrange an appointment using our contact details at the bottom of the page.

GPs or other health professionals can refer children to us using the professional referral form.

What we do

Imms serviceWe deliver the routine national immunisation programme to school-aged children whether they are in school or home-educated. We offer vaccinations at your child’s school or in community clinics.

Vaccines are the most important thing we can do to protect ourselves and our children against ill health now and in the future. The NHS has a guide to help you understand the vaccines offered in the UK and when to have them. The NHS website also explains how they work and why they're safe. You can find out more about the vaccines offered to your child before school, and when they are at primary school and secondary school on our website.

Families who might need further support

We understand children with special educational needs, a disability or those who have a needle phobia may find vaccination sessions stressful. We would like to offer all children the opportunity to receive their vaccinations in an appropriate and comfortable environment.

  • We can go to your child’s school and vaccinate – you may wish to be with them, but this is not a requirement.
  • We have regular community clinics across Kent and Medway that vary from children’s centres to hospitals. You can book at a location and time of your choice and come along with your child.
  • In exceptional circumstances, we may be able to offer home visits.

My son suffers with severe anxiety and was extremely nervous whilst at the appointment. All 3 nurses were absolutely incredible with my son. They immediately put him at ease and explained everything to him whilst keeping him calm and reassured. I can’t thank these amazing ladies enough for the care and attention they gave to my son.
